Florida School Crossing Guard(s) of the Year

To acknowledge superior accomplishments by local School Crossing Guards the Florida Department of Transportation (FDOT) annually recognizes a Florida School Crossing Guard(s) of the Year.

The first Friday in February has been traditionally celebrated as School Crossing Guard Appreciation Day. On this special day, selected Crossing Guard(s) exemplifying professionalism and responsibility for the safety of children are recognized.

If you know a Florida School Crossing Guard who deserves this recognition, please complete the nomination form. The nomination window closes on December 25, at 11:59 p.m. (EST).

Crossing Guard Nomination forms (Free Adobe Acrobat PDF Reader) may be submitted via e-mail to cgadministrator@dot.state.fl.us, via fax at 850-414-4221 or mail/deliver to (Attention: FSCGP) FDOT, Safety Office, 605 Suwannee Street- MS #53, Tallahassee, FL 32399-0450.

You do not have to use this form, but consider the criteria outlined in the form when nominating a guard for consideration as guards are scored on this criteria. Please write a minimum of 1 sentence for each for each criterion.

More than one page may be used, and supporting documents (letters of recommendation, media coverage, or any other document or photo that supplements the application or is too difficult to summarize within the application itself) are welcomed. All fields are mandatory unless otherwise noted as optional.
